Online Intake Agreements
By completing an online intake form (the “Agreement”), you acknowledge and agree to the following:
You are the named insured or otherwise authorized to act on behalf of the policyholder for the property involved in this claim, and the reported damage occurred during a period when insurance coverage was in effect.
You agree to carefully review the Agreement and all of its provisions before electronically signing the document.
Submission of the Agreement does not automatically create an attorney-client relationship. Representation by JT Law Firm begins only after a consultation with our team has taken place and the Agreement has been accepted and countersigned by JT Law Firm.

you receive from us. After opting out, you will receive a confirmation message, and we will cease sending SMS messages to your number.

Privacy Policy
• Your phone number will be handled in accordance with our Privacy Policy. We do not sell or share your
phone number with third parties except as required by law.
